A deeper evaluation looks beyond total cholesterol and LDL cholesterol alone to better understand the factors that influence plaque development, arterial health, and long-term risk. Advanced assessments such as Apo B, one-hour glucose testing, and carotid intima-media thickness (CIMT) testing can provide additional insight.

Apo B testing measures the number of atherogenic lipoprotein particles circulating in the bloodstream. These particles, which include LDL and other cholesterol-containing particles, can contribute to plaque formation when they enter and interact with artery walls. For some individuals, Apo B may reveal elevated risk even when a standard cholesterol panel appears acceptable and lower Apo B may reveal lower risk when total cholesterol appears elevated.

One-hour glucose testing provides information about how the body responds to a glucose challenge. Standard fasting glucose testing may appear normal while early metabolic changes are developing. An exaggerated rise in glucose after a challenge can provide insight into insulin sensitivity and metabolic health, which are important factors connected to long-term cardiovascular risk. False negatives, (missed cases of impaired glucose tolerance) are twice as frequent using the more common tests of Fasting Glucose and HbA1C compared to the One-Hour Glucose Tolerance Test. The studies show that the OGTT more accurately assesses glucose intolerance and diabetes and heart disease risk. Some cardiovascular researchers and clinicians consider glucose intolerance to be an important marker of cardiovascular risk, alongside factors such as Apo B and inflammation.
